Seven supplied chapters are not all 12 core functions.
The source set contains Sections 1–7. This build does not claim that the remaining five chapters were supplied or completed. New clinical policies and precise professional/payer requirements need their own controlled review.
Use the Golden Thread without erasing the source history.
The older chapters describe Assessment → Plan → Note and related variations. The Builder specifies Assessment → Meaning → Problem → Goal → Objective → Intervention → Evidence → Review/Transition. The expanded chain is attributed to the Builder rather than presented as identical wording in every earlier manual.
