Source-model reference — not an announcement of an active treatment service.

CW360 / DESIGN LIBRARY

A referral is a beginning, not an ending.

The CW360 source standard keeps responsibility visible until the next state is known: notice, acceptance, scheduling, confirmation and tracking—or an honest disposition.

Notice → Accept → Schedule → Confirm → Track

The Builder states this sequence as its handoff standard. The workflow chapters add documentation, permission and follow-up ownership.

StateWhat the working record should make visible
NoticeThe need or referral has been received and assigned
AcceptThe receiving party’s response is known
ScheduleThe next appointment or action is set, or its barrier is documented
ConfirmReceipt, attendance or the next completed event is verified
TrackRemaining responsibility, follow-up and final disposition stay visible

Closure does not mean forcing an outcome.

The case-management chapter identifies linked, pending, declined and closed-unable dispositions. A refusal, waitlist or unresolved barrier should be recorded truthfully with any remaining follow-up responsibility.

DispositionSource meaning
LinkedAppointment attended or service started
PendingWaitlist or processing continues
DeclinedThe person chose not to proceed; reasons recorded when shared
Closed-UnableBarriers prevented completion; alternative plan documented

A public website is not the clinical handoff record.

This site describes the process. Actual client records, releases and clinical communication belong in the organization’s approved systems; the business inquiry channel is not used to collect them.

Bring the model into real operation.

Define the receiving organization, authority, scope, people, tools and review evidence before adopting or launching a service.

Source basis: Builder System Instructions, Clinical Integrity; Section 7 Case Management §7.5; Best Practice Guide §11 Website adaptation; source-model details are not an announcement of service availability.
