The optional business inquiry form requests a contact name, organization, work email, and a selected business topic. It has no attachment field or open message box. Do not submit client information, health information, identification numbers, clinical records, or other confidential material.

When enabled, the form sends the submitted business contact details to the destination configured by the site administrator. The CW360 plugin does not store the form submission as a WordPress record. Email systems, hosting systems, security tools, and ordinary access logs may retain information according to their own configurations.

Technical data and cookies.

The supplied theme does not add advertising trackers or external font requests. WordPress, the hosting service, and additional installed plugins may use cookies, logs, security controls, and other technical processing. Brief rate-limiting counters are used when the inquiry form is enabled; the CW360 plugin uses a salted hash rather than saving the raw address in that counter.
